Jack Hoffmann, LCSW is a licensed clinical social worker at Eastern Long Island Hospital who serves in the capacity of Behavioral Health Director for Provider and Clinical Relations. His clinical experience as a counselor and intake coordinator for Quannacut Inpatient Rehab and Detox have given him in depth insights into the field of addiction services . He also maintains a private counseling practice in Sag Harbor. Jack presently serves as Vice Chair of the Suffolk County Heroin Opiate Advisory Panel created<br>in 2010 to better address the treatment options for youth who are addicted to heroin and opiates and make recommendations for improving access to prevention, treatment and recovery services in Suffolk County.
